The Patient Profile

The Patient Profile provides valuable information and context to support your care coordination workflows. The Profile enables you to review your patients' demographic details, Care Team information, Care Instructions, Recent Post-Acute Providers, Care Summary, Encounter History and HHA Coverage History.

Demographic Information

The demographics section of the Patient Profile includes the following information:

First name Last name Age Gender DOB Patient ID Managing Provider Insurance

Currently billing Encounter Insurance(s) Last 12 Months Patient phone Address SSN Bamboo Health flags

Care Team and Care Instructions

These sections provide further details and contact information for your patient's care team, including:

1. Care Coordinators
2. Providers
3. Programs
4. Practices

Care Team information and Care Instructions are provided directly by your organization and/or other Managing Organizations responsible for your patient's care. Please contact your organization's account administrator to update any care team details listed in your patients' profiles. 

Recent Post-Acute Providers

The Recent Post-Acute Providers section displays the post-acute facilities where your patient has recently received care within the Bamboo Health network.

Care Summary

---

Care Summary  includes the Encounter Summary and Diagnosis Summary. To see more granular information on either of these topics, simply expand your view by selecting the blue caret:

Please note that you can filter the diagnosis summary by the following time periods: Last 30 days, Last 3 months, Last 6 months, Last 12 months, All-time.  If there are fewer than 10 results, all diagnosis categories will be shown by default. 

Encounter History

The Encounter History is organized chronologically and provides a longitudinal view of your patients’ prior care encounters across the Bamboo Health community.

Your patients’ open and ongoing encounters will be highlighted in yellow to the left, and will also be fully expanded. You can expand any prior event by clicking on the blue arrow to the right of the encounter, or you can move your cursor to the top of the encounter history and click on Expand All to get more information.

HHA Coverage History

Your patient’s ongoing and prior home health services will be separated from and shown to the right under HHA Coverage History.
