1. Saved Filters in Pings: Identify Patients Receiving Care in the ED or Urgent Care
Why: Identify patients receiving services in the ED, or Urgent Care in real-time
How:
- In the filter pane on the left of your dashboard, choose the following filter criteria:

- Current status: Presented and Visit

- Once you’ve selected the filter criteria, click create, name, and save your filter.
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
2. Saved Filters in Pings: Identifying 3-Day Waiver Eligible Patients
Why: Managing patients eligible for the 3DW supports admission to a SNF directly from the ED or Observation. When clinically appropriate, the ACO can facilitate the transition to a SNF without the requisite 3-day inpatient stay. Identifying these patients can support both improved patient outcomes and decreased total cost of care.
How:
- In the filter pane on the left of your dashboard, choose the following filter criteria:
- Setting: Emergency and Observation
- Program: Your organization’s priority care program
- Once you’ve selected the filter criteria, click create, name, and save your filter.
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
3. Identifying High Utilizers
Why: This flag identifies patients who may benefit from a check-in from your population health management team regarding their ED utilization patterns and education on other, more appropriate sites of care for future care events.
How:
- In the filter pane on the left of your dashboard, scroll down and check off high utilizer under the Bamboo Health Flags header
- Scroll up to save and name your filter; after you’ve entered the name of your filter, be sure to click save
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
4. Identifying Recent SNF Stay Patients
Why: This flag highlights potential opportunities to discharge patients to SNF care without an additional three-overnight hospital stay. It also informs the hospital staff of which SNFs are discharging back to your hospital.
How:
- In the filter pane on the left of your dashboard, scroll down and check off recent SNF stay under the Bamboo Health Flags header.
- Scroll up to save and name your filter; after you’ve entered the name of your filter, be sure to click save.
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
5. 30-Day Readmission Management
Why: This flag identifies opportunities to reduce potential readmission penalties. Filtering your dashboard to patients with the readmission risk flag allows you to intervene in real-time with treating providers in the ED or Observation. When clinically appropriate, patients identified under the readmission risk flag may require additional care coordination, ambulatory care, and/or community-based services to prevent readmission.
How:
- In the filter pane on the left of your dashboard, choose the following filter criteria:

6. Identify Patients by Length of Stay
Why: Creating saved filters by SNF LOS in Pings expedites efforts to focus on patients by their LOS. Taking steps to reduce LOS can support your organization’s efforts to a. meet an average LOS goal, b. decrease overall cost of care for patients in risk-based contracts, and c. improve patient outcomes and satisfaction.
How:
1. In the filter pane on the left of your dashboard, choose the following filter criteria:

- Current status: Admitted
- Facility type: Skilled Nursing Facility
- Setting: Skilled
- Visit duration: Your priority length of stay range
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
7. Customize Your Text and Email Alerts to Your Top-Priority Care Programs
Why: Identify your admitted patients who are attributed to an ACO or other care program.
How:
- In the filter pane on the left of your dashboard, select admitted under the current status header. Scroll down to the program header and select your priority ACO/care program.
- Scroll up to save and name your filter; after you’ve entered the name of your filter, be sure to click save.
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
8. Customize Your Text and Email Alerts to Your Top-Priority Hospitals
Why: Receive real-time email and text alerts on your patients’ care events at specific hospitals.
How:
- In the filter pane on the left of your dashboard, click the blue carrot next to the facility name filter header to expand it.
- From here, select your top-priority hospitals either by checking them off from the first five options listed; by using the search bar to find them by name; or by clicking show all options to select from all possible selections.
- If you’d like to receive notifications only for specific categories of events at these hospitals (eg admissions only), choose the applicable options from the current status filter as well.
- Once you’ve selected your top-priority hospitals and statuses, scroll up to name and save your filter; after you’ve entered the name for your filter, be sure to click save.
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
9. Identifying Patients with the Diagnosis Associated with COVID-19 Flag
Why: This flag provides better transparency and insight into your patients with a diagnosis associated with COVID-19, allowing you and your team to determine care coordination next steps, and run reports to inform your COVID-19 management efforts.
How:
- In the filter pane on the left of your dashboard, scroll down and check off COVID-19 under the COVID-19 header.
- If you’d like to receive notifications only for specific categories of events (e.g hospital events, SNF events etc.), choose the applicable options from the facility type filter as well, or focus on a specific status (e.g. discharges), using the current status filter facet.
- Once you’ve selected the filter criteria, click create, and be sure to save and name your filter.
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
10: Identify Outside Hospital Events
Why: Identifying outside hospital events ensures you never miss important patient care events and enables learning opportunities for potential workflow improvements to increase patient retention within your organization.
How:
- In the filter pane on the left of your dashboard, check off admitted under the current status filter header.
- Click the blue carrot next to the facility name filter header to expand it. Select the x next to your organization’s name to omit events within your organization.
- Under the facility type filter header, check off hospital.
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
11. Telehealth Opportunities with Saved Filters and Notifications
Why: Identifying patients who qualify for telehealth opportunities helps improve your team’s clinical workflows and can lead to improvement in patient engagement following their discharge from the acute or skilled setting.
How:
- In the filter pane on the left of your dashboard, under the current status filter header select discharged.
- In order to focus on your patients’ hospital or skilled nursing facility events, select hospital and/or skilled nursing facility under the facility type filter header.
- Once you’ve selected the filter criteria, scroll up to name and save your filter; after you’ve entered the name for your filter, be sure to click save.
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
12. Identifying Patients Who May Be Eligible for Transitional Care Management
Why: TCM visits provide the opportunity for a PCP or Specialist to perform medication reconciliation, educate on self-care/disease management, care plan development and/or address patient questions, all while assessing any risks for readmission. Transitional care can help improve quality outcomes, access, and patient satisfaction, provide the opportunity for coordination across the care team, and allow a provider to be reimbursed for care already being provided following discharge.
How:
- In the filter pane on the left of your dashboard, expand the current status filter header by clicking the blue carrot next to it. From here, select discharged by checking off the box.
- Next, click the blue carrot next to the facility type filter header and expand to select Hospital, Skilled Nursing Facility, and Rehab Hospital.
- Under setting, select the x next to emergency; this will exclude patients with ED encounters.
- Find the event date filter header and enter 3 days ago to ensure you don’t miss any weekend discharges.
- If you and your team specifically follow certain cohorts, payers, or populations of patients, you may need to further refine this filter by the specific programs.
- Once you’ve selected the filter criteria, click create, and be sure to save and name your filter.
- Turn on filter notifications via email or text to receive real-time alerts on these events!
- Your new filter will now be listed under My Saved Filters!
